● “AIC Stories Blog Survey 2021” Giveaway is proudly brought to you by Agency for Integrated Care (AIC).

● The giveaway will end on 10 February 2021, 2359hrs.

● Survey respondents must be 18 years and older and residing in Singapore to be eligible for receiving the mask holders.

● Survey respondents must provide us with their mailing address in full to receive the mask holders. We regret to inform you that AIC will not be able to mail out any mask holders to mailing addresses that are incomplete.

● A maximum of one (1) submission per person is allowed for this survey. Where a participant submits two (2) or more entries, the first submission will be accepted, and the rest of the entries will be void.

● All survey respondents who are eligible will receive two (2) AIC mask holders.

● AIC will not accept responsibility for gifts lost, delayed, misdirected, damaged or undelivered.

● AIC reserves the right to change the terms and conditions without any prior notice.
